  • Visiting clients in their homes with above precautions and social distancing
  • Visiting clients in facilities by means of a "window" visit when possible/appropriate
  • Working to get telephonic medical updates on clients in facilities
  • Attending care plan meetings via phone/zoom
  • Attending doctor's appointments via telehealth or in person if/when the doctor's office is open
  • Advocating for the best quality of care either by phone or in person
  • Following up with other providers and health care matters as usual
  • Providing assistance with placement/relocation when necessary
  • Providing caregiver support and strategies via phone or zoom meetings
  • Shopping for needed items for clients (essential goods such as food, paper products, and medications)
